The script Form Handler is a Form Processors script (process any kind of forms).
form_handler() cretaes a list of all form data submitted by any form. The output can then be emailed to a sites ssytem administrator, or saved to a file.
form_handler_generator() can assist you in the making of a more customized sloution for any particular form. Just remmeber to check your checkboxes or they will not be noticed.
